Up to $4,000 in the event of outpatient surgery or fracture.

Don't let an ambulance ride alarm you, with up to $800 in reimbursement for an ambulance.

Up to $40/visit for rehabilitation therapy such as physical, speech or occupational therapy.

Up to $200 back for items such as crutches, wheelchair, walkers, nebulizers, etc.
